How can I refer a patient to Mountbatten Isle of Wight's services?

We encourage health professionals to refer patients who may benefit from our care as early as possible. This allows us to provide timely support and plan for their future care needs through Mountbatten's Coordination Centre. Click HERE to find out more.

How can Mountbatten Isle of Wight support health professionals in caring for patients? 

At Mountbatten Isle of Wight, we work closely with health professionals to provide expert support for patients with advanced, life-limiting illnesses. Below you’ll find resources, tools, and guidance to help deliver high-quality palliative care.

Just in Case drugs
"Just in Case" drugs are recommended for patients with advanced, life-limiting illnesses where the prompt management of foreseeable symptoms is essential. This is particularly important for patients in their last months of life.

For GPs and Community Health Professionals:

  • Use SystmOne: Locate the form by selecting "IoW palliative" or searching in the tree. Select the required drugs, generate the FP10, and choose "community administration" to transfer it to the Just in Case permission-to-give form.
  • Alternatively, Click HERE to download the Just in Case Drugs form.

For hospital clinicians:

  • For inpatients: Prescribe the medicines outlined on the Just in Case drugs form using JAC.
  • For patients going home: Complete both a TTO (for pharmacy) and the form to authorise community nurses to administer the specified medicines as needed.

Pain assessment in cognitive impairment
The PAINAD tool is designed to help assess pain and monitor the response to analgesia in patients unable to verbalise their pain.
