At Mountbatten Isle of Wight, we are dedicated to supporting people with life-limiting conditions and their families, offering care that allows you to live well and plan for the future.

Our Mountbatten Coordination Centre is here to support people who are expected to live for five years or less, whether due to cancer, heart failure, dementia, Motor Neurone Disease, or frailty due to ageing. We aim to provide care and support earlier in the process, helping people live as well as possible.


How to access Mountbatten Isle of Wight's services

Our services are completely free, and the process is simple. Ask your GP or healthcare professional to refer you to our Mountbatten Coordination Centre. 


What happens once you are referred to Mountbatten Isle of Wight's Coordination Centre?

Once referred, you’ll be placed on our Share My Care register (with your consent), and a Mountbatten Nurse will assess your needs. Our team works closely with health and social care professionals to ensure you receive coordinated, personalised care that fits your needs.

24/7 support when you need it

Our services include access to a 24-hour advice and support line on 01983 533331. This line provides reassurance, helps prevent unnecessary hospital admissions, and is always available for any questions you may have.


Planning for the future

We encourage everyone to complete an Advance Care Plan. This plan fosters an important discussion about your preferences for care, particularly if you are unable to make decisions in the future due to illness. By understanding your wishes early, we can ensure your care meets your needs. For example, many of our patients prefer to be supported at home by our care at home team.
